Well, that’s something I’m still trying to feel out.  Because I think that the only reason that worked in canon is because it was a smaller, symbolic representation of what the prophecy intended on the larger scale.  Ardyn’s (double, technically, and I have a feeling the second death was the more important one, the one in the Beyond) death was important only in the sense that it functioned as a microcosm of the destruction of the Scourge.  It’s also why I think the Astrals either actively deceived Ardyn in the ancient past or didn’t really know what they were doing initially, but that’s another issue.

So theoretically, the Scourge in the world would be fading away just as it does in Ardyn.  It’d be much slower, though, because it’s still an infectious disease and there would be new cases.

And I think initially Ardyn becomes something like an asymptomatic carrier as the Scourge is very, very slowly destroyed by the Crystal.

The prophecy comes into play more because Shiva and Bahamut aren’t satisfied with this version of events—either through impatience, hubris, or both—and are trying to push the prophecy through to its conclusion while Ardyn still technically has the Scourge.  They have a time limit now, so they’re actively interfering, and in the process they’re really pissing everyone else off.

Especially since even without Scourge symptoms, Ardyn isn’t aging due to the Astrals’ Curse on him.  And Noctis and Luna keep having inexplicably serendipitous and/or dangerous encounters that seem a lot like particularly heavy-handed divine herding.

The parents involved are very upset by the latter in particular.
